Medical, Prescription and Vision Insurance
Employees and their eligible dependents can choose from the following 3 medical insurance options. All of them include vision and prescription coverage.

Employees can choose from a national network of Blue Cross Blue Shield providers.
- Monthly employee cost:
- $42 for individual employee.
- $86 for employee + spouse (or dependents).
- $120 for family.
- Annual deductible: $250 per individual, $500 maximum per family.
- Prescription benefits are provided through Express Scripts.
- Vision benefits provided through VSP.
Employees choose a primary care physician and obtain all services through Kaiser Permanente.

- Annual Deductible: $250 per individual employee, $750 maximum per family
- Prescription and Vision benefits provided through Kaiser Permanente.
High Deductible Health Plan administered through Blue Cross Blue Shield for medical and prescription coverage. Vision coverage is provided by VSP.
- Monthly employee cost:
- Health Savings Account is administered through EBDS bank. In addition to pre-tax payroll contributions made by employees, OSM contributes $750 per individual employee and $1,500 per family.
- Annual Deductible: $1,500 per individual employee, $3,000 per family
- Prescription discounts through Medco.
Dental Insurance
Employees and their eligible dependents can choose between 2 dental plans regardless of their choice of health coverage.
Employee chooses from a network of providers.
- No copays for office visits.
- Plan pays 100% of preventative and 90% of basic restorative services.
- Annual Deductible = $50 per individual employee, $150 maximum per family.
- Calendar Year Maximum $2,000 per person.
Employee obtains services through Kaiser Permanente.
- $5 copay per visit.
- $25 copay for Emergency/Urgent Services.
- Plan pays 100% of preventative and basic restorative services.
- No Annual Deductible.
- Calendar Year Maximum: $2,000 per individual.
Flexible Spending Accounts
These accounts allow employees to use pre-tax money to pay for family medical, dental, and vision expenses not covered by insurance, e.g. deductibles and co-payments, up to $5,000 per year. They can also be used for child/eldercare expenses up to $5,000 per year.
- Expenses may be incurred until March 15th of the year following the plan contribution year.
- All claims must be submitted for reimbursement before May 15th or funds will be forfeited according to Federal Law.
- These accounts are managed through Flex Plan Services.
- Use the BennyTM Debit Card to pay your provider directly for qualified medical care expenses.
